On the Children’s Oncology Group site, you can search the “Late Effects Directory of Services” by plugging in your state or the nearest major city. It’s important to only search by institution or city or state: plugging in too many variables can result in no returns.
The clinicians who submit their COG-affiliated institution’s name to the directory include eligibility criteria (for example, if there is an age cut-off) and contact information for who survivors should reach out to regarding appointments and new patient enrollment.
